Altered states of consciousness are variably described using such terms such as coma, obtundation, stupor, confusion, and semi-coma. These states are comprised primarily of two components: - arousal, and - content of consciousness, and these may be differentially affected. In the assessment and management of patients suffering from these states, the cardinal principle should be that altered consciousness is a SYMPTOM of underlying brain disease and is not a primary diagnosis. Information must be gathered from the history, physical and neurological examination and laboratory which give clues to the underlying diagnosis. This manual outlines the most important and common cause of altered consciousness to aid rapid diagnosis and appropriate therapy (Fig. 1).
